Hemodynamic changes in prone position - a non‑invasive physiological study

چکیده
CĂl studie: CĂlem tĂŠto fyziologickĂŠ studie bylo sledovat zmÄny srdeÄnĂho vĂ˝deje a dalĹĄĂ hemodynamickĂŠ parametry v pronaÄnĂ poloze srovnat vliv podloĹženĂ hrudnĂku pĂĄnve na parametry. Typ ProspektivnĂ, observaÄnĂ studie. pracoviĹĄtÄ: ExperimentĂĄlnĂ laboratoĹ fakultnĂ nemocnice. MateriĂĄl metoda: Do zaĹazeno 12 zdravĂ˝ch dobrovolnĂkĹŻ starĹĄĂch 18 let. V supinnĂ (poloha S) zahĂĄjeno neinvazivnĂ monitorovĂĄnĂ systĂŠmem ClearSight/EV 1000. Po stabilizaci byly zaznamenĂĄny: srdeÄnĂ index (CI), tepovĂŠho objemu (SVI), variabilita (SVV), systĂŠmovĂŠ vaskulĂĄrnĂ rezistance (SVRI). DĂĄle definovanĂ˝ch mĂstech sonograficky sledovĂĄny: exspiraÄnĂ plocha v. cephalica (sVCe), saphena (sVSe), jugularis interna (sVJe), inspiraÄnĂ (sVCIe sVCIi) maximĂĄlnĂ minimĂĄlnĂ pĹedozadnĂ prĹŻmÄr (dVCImax dVCImin) cava inferior kalkulovĂĄn kolapsibility (VCI CI). Sonograficky byl stanoven corrected carotid flow time (ccFT). MÄĹenĂ byla opakovĂĄna nepodloĹženĂŠ P1) podloĹženĂŠ s podporou proximĂĄlnĂ ÄĂĄsti P2). VĂ˝sledky: Mezi jednotlivĂ˝mi polohami nebyly zjiĹĄtÄny rozdĂly hodnotĂĄch CI, SVI, SVV ccFT. signifikantnĂ MAP VCI CI. Ve srovnĂĄnĂ polohou S P1 pozorovĂĄna vyĹĄĹĄĂ hodnota SVRI sVJe. NiŞťà dVCImin zaznamenĂĄna P2 ve P1. ZĂĄvÄr: U nesedovanĂ˝ch nebyl pozorovĂĄn pokles ani preloadu, ale stagnace krve jugulĂĄrnĂm ĹeÄiĹĄti. zjiĹĄtÄna zvýťenĂĄ kolapsibilita dolnĂ dutĂŠ ĹžĂly.
